Hospice Services case managers/nurses work with patients, their families, and other healthcare professionals during the final stages of patients’ lives. Our nurses work in private homes and residential care facilities. Our nurses must be capable of compassionate communication with patients and families, have keen observation skills, confidently care for patients and have good time management.
How you will make a difference:
- Symptom Management: Through a thorough assessment, you will work closely with the patient’s primary provider and the Hospice Medical Director to ensure each patient’s symptoms are well managed.
- Plan of Care: Evaluate the treatment and medical condition of the patient according to the plan of care. Individualize the plan of care and adjust as needs change.
- Continuity of Care: Manage interdisciplinary care as applicable while promoting continuity of care with appropriate admissions, transfers and discharges.
- Family Educator/Trainer/Advocate: Counsel patient and family on the disease and its progression and incorporate patient and family in development of plan of care. Listen to patient and family members.
- Patient-Centered Team: Work with an interdisciplinary team who cares deeply about delivering the right care at the right time for every patient.
